Vision screenings and examinations 

Vision screenings and exams must be completed by students in the grade levels or groups listed below before October 15 of the current school year: 


CPS has certified vision technicians on hand at each school to provide screenings. Test results are provided to the parent only if follow up is required.


Vision examination 

In Illinois, Public Act 95-671 requires that kindergarteners and all students enrolling for the first time in a public, private or parochial school must have an eye examination. The requirement went into effect January 1, 2008.

Exams must be completed by a licensed physician or a licensed optometrist. If the student fails to present proof of exam by October 15 of the current school year, the school may hold the student’s report card.

Ask your child’s physician to complete his or her vision exam form. Once completed, return the vision form to your child’s school. This form is proof that your child has completed his or her vision exam.


Vision clinics

CPS, in collaboration with OneSight, Illinois College of Optometry, Community In Schools-Chicago and the Lion’s Club, provides eye exams and eyeglasses to students during various vision clinics throughout the school year. Services are provided at no cost to the students. Students who have failed previous vision screenings or are referred to us by a teacher are invited to attend. Contact your child’s school to find more information about the vision clinics.

If your child’s school has been invited to participate in a vision clinic, this is the required consent form. Vision clinic consent forms can be downloaded in either  English or  Spanish.
