Paper Applications


Paper applications for the 2017-2018 application process are below. Please note the following information for non-CPS students.

NWEA MAP Registration for Non-CPS Students

If your child is applying to grades 5-9, did not attend a Chicago public school or charter school last year, and is applying to Selective Enrollment Elementary or High Schools, IB High Schools, Magnet High Schools, Military Academies, or CTE-College and Career Academies for grades 5-9, you must register your child for the NWEA MAP. If you have not already done so, click here to access the registration form, which must be submitted by December 9, 2016, along with paper applications (see below) for the programs in which you are interested. If you are registering for the NWEA after September 9th, you will not be able to apply using the online process and you will not receive a PIN.

Online Application

The online application and scheduling website allows parents/guardians to apply online instead of using paper applications for schools and programs that are managed by the Office of Access and Enrollment: magnet elementary schools, magnet cluster schools, elementary open enrollment schools, magnet high schools and programs, Selective Enrollment Elementary Schools, Selective Enrollment High Schools, International Baccalaureate High Schools, Military Academies, and CTE-College and Career Academies.

 

First Grade Exception Form


Students must be six years old by September 1, 2017, in order to apply for first grade enrollment for the 2017-2018 school year. However, if your child will become six years old between September 2, 2017 - December 31, 2017, he/she may qualify for an exception to the age requirement for first grade enrollment.

To qualify for this exception, your child:

  1. Must become six years old between September 2, 2017 - December 31, 2017
  2. Must have attended preschool and kindergarten at the same nonpublic school, or the same public school outside Illinois
  3. Must have been taught in kindergarten by an appropriately certified teacher
  4. Must be deemed ready by the nonpublic school (or the public school outside Illinois) to work at the first grade level

If you are applying for first grade enrollment for your child under this age exception, you must apply via paper application and you must submit a First Grade Age Exception Form to the Office of Access and Enrollment along with your application.

NOTE: If you are exercising the first grade age exception, and you are submitting both the Standard Elementary Schools application and the Selective Enrollment Elementary Schools application, you must apply to first grade on both applications; you cannot apply to first grade on one application and to kindergarten on the other.
