Nepotism Certification Form
A Nepotism Certification Form
should be completed by a principal, assistant principal, or other CPS manager to certify that a subordinate employee is not his or her relative. This form should only be completed in response to a complaint or inquiry brought to the Ethics Advisor.
Secondary Employment Approval Form
A Secondary Employment Approval Form
should be completed and filed by employees who work 52 weeks per year and also perform non-CPS employment or activities for any type of compensation. This form also should be completed by school-based employees working less than 52 weeks per year who have secondary employment during the school year. This pertains to both 52-weeks-per-year employees as well as school-based employees working less than 52 weeks per year who have secondary employment during the school year. Approvals should be granted unless the proposed secondary employment is during the employee’s scheduled work hours, or there is a conflict between the secondary employment and the employee’s Board employment.
Intersession Secondary Employment Notification Form
An Intersession Secondary Employment Notification Form
should be completed and filed by employees who work less than 52 weeks per year and engage in non-Board employment during intersession. Secondary employment during intersession does not require approval, but it still must be reported to the Ethics Advisor.
Statement of Business and Financial Interests
The Statement of Business and Financial Interests (SBFI)
is an annual financial disclosure required of all Chicago Board of Education members and designated employees. The statement, like taxes, covers the former calendar year. SBFIs are submitted electronically and due on May 1 of each year. A link to the most recent form will be sent to those employees who are required to file an SBFI, with ample time to complete and file the form.
Ethics Travel Certification Form
An Ethics Travel Certification Form
is required when a Board Vendor, prospective Board Vendor, or event organizer pays for all or part of an employee’s travel or participation in any event off CPS premises. Ethics review is not required when travel costs are paid by the employee, school fundraising, a grant, or CPS.
Travel requests are submitted through the Department of Finance’s Oracle travel approval process. A signed Ethics Travel Certification Form and a conference brochure, invitation, or other travel documentation should be scanned and uploaded into the attachments section of the Oracle travel submission. The travel approval request should be submitted in Oracle prior to making any travel arrangements and well in advance of the travel date. The Ethics Advisor and the Ethics Committee will review the travel description, estimated cost details, and attachments for Code compliance.
